Community Healthcare Center is a primary medical, dental and counseling clinic welcoming all ages from newborns to seniors. The medical, dental, and counseling providers are committed to providing superior primary and preventive care for your entire family. A variety of patient services are offered including Pediatrics, Obstetrics, Family Medicine, Children & Adult Dentistry, Behavioral Health Counseling for Children & Adults, and Today Care. They also have pharmacy, lab, and x-ray.
Community Healthcare Center can provide citizens with cholesterol screening tests, including a lipoprotein profile. The results will be interpreted by a primary care provider. The onsite laboratory assures fast results in 1- 2 business days.
Remember you must be fasting for the lipoprotein profile.
Uninsured and underinsured patients can benefit from financial assistance programs that are available for health situations you may be experiencing.
If you have private or employer-based insurance, the clinic welcomes that too.
A sliding fee scale is available for all qualified clients without medical insurance. The sliding fee scale may also apply to services which an insurance does not cover.
No one is denied services due to an inability to pay. Please ask the staff if you need assistance or have any questions.
Service co-pays required at the time of service. See the receptionist at the time of service for the amount due.
Today Care
Community Healthcare Center’s Today Care is a same day walk-in primary care clinic that is open to current patients of Community Healthcare Center (CHC).
New patients are welcome that choose to make CHC their “provider of choice.” HOWEVER due to COVID, the clinic is only accepting new patients out of the following categories:
- 21 years old or younger
- Pregnant Women
- 65 years old or older
What should you bring to your walk-in appointment?
• Insurance card (if you have health insurance)
• Picture ID
• Social Security card
• Immunization record
• Proof of household income
• Bottles of all medications you are currently taking.
