Diabetes Self-Management Outpatient Program at United Regional

United Regional’s Diabetes Survival Self-Management Outpatient Program has been certified by the American Diabetes Association for providing an exceptional program. This program is taught by Certified Diabetes Educators and a Registered Dietitian.  Classes are individualized to YOUR needs and usually can be completed in 3 to 4 sessions. Upon completion of the program, follow up visits provide an opportunity for patients to meet with diabetes educators to assess and review progress in managing diabetes. The “self-management” concept means that YOU are the central focus, and your “team” includes:

  • Your physician
  • Certified diabetes educator
  • Registered dietitian
  • Registered nurse
  • Many other health care professionals

This program is for anyone who has diabetes including:  Type 1, Type 2 or gestational diabetes (diabetes in pregnancy). This program includes:

  • Community resources
  • Continuous glucose monitoring
  • Diabetes overview
  • Exercise education
  • Glucose monitoring and interpretation of results
  • Goal setting and problem solving for daily living
  • Insulin initiation
  • Insulin pump initiation and follow up
  • Management of diabetes before and after pregnancy
  • Managing low & high blood glucose
  • Meal management
  • Medications
  • Prevention of complications
  • Psychosocial adjustment
  • Sick day management
  • Stress management
  • Support groups

The Diabetes Self-Management program also offers a monthly nutrition class for patients who have attended the one-day educational class.  This education class is taught by certified diabetes educators and registered dietitian and focuses on:

  • Managing your lipids (cholesterol and triglycerides)
  • Menu planning and grocery shopping
  • Dining out
  • Benefits of fiber

How to get started:

In order to enroll in the program, a physician referral is required.  Once the referral is received, the staff will contact you regarding your appointment.
