Share the Care Program

This program is only for Community Health Center and Family Health Center patients. To apply for this program one must be a patient and one must first meet with eligibility. There may be a $15 enrollment fee to apply for prescription assistance. The Community Healthcare Center services 13 counties in the North Texas Region.
